Transcript Release

Transcripts from Mineral Wells High School may be released upon written request from:

• The former student, provided s/he is 18 years of age or older, or
• A parent/guardian, provided the former student has contacted us and given permission to release the transcript

The requestor must provide a photo state-issued ID or a current driver’s license.

Note: Official transcripts will be released in a sealed envelope. Transcript must remain in the sealed envelope to be official.
